LifeQuest uses a team approach in providing services to people with developmental disabilities.  The person receiving services and his/her family are members of the team.  Other members may include the person's guardian, advocate, staff from LifeQuest, and consultants from other agencies, if appropriate.  Each person receiving services has a service coordinator who is also a member of the team.

The service coordinator, a primary advocate for the person, ensures that the person's needs are identified and addressed by the team and/or community agencies.  The service coordinator is responsible for the facilitation of an annual individual service plan (ISP) meeting for the person.  During the ISP, the team identifies strengths, needs, goals, and objectives, and develops an individualized service plan.  The service coordinator is responsible for monitoring the service plan for progress and assuring that the service plan is being implemented as identified.

The service coordinator also serves as the primary contact for family members, guardians, and advocates.  Other responsibilities of the service coordinator include the facilitation of all team meetings, assisting the person with community integration, funding eligibility, benefits, financial, obtaining health services and/or other professional services, and locating and maintaining affordable housing.
