The following basic criteria will be used to
evaluate a person's eligibility for services at LifeQuest:
- The person must be 16 years of age, or
older. Younger people may be considered, if the Admission Committee
decides that LifeQuest is the most suitable placement for the person.
- The person must have a developmental disability.
A diagnosis must be available, in writing, from a physician, psychologist, or
other appropriate professional.
- The person must be eligible for an appropriate
funding source (government entitlement program, rehabilitation services,
special education, etc.) or able to purchase services with his/her resources.
