Any person with a developmental disability who has met the eligibility criteria listed below will be considered to receive services from LifeQuest.  Referrals to LifeQuest for services may be initiated by the person, family members, guardians, advocates, other Agency personnel, school districts, or other interested people from the community.

LifeQuest has an Admission Committee.  The Admission Committee reviews all applicants who wish to obtain services from LifeQuest.  Functions of the Admission Committee are as follows:

1. The Admission Committee Chairperson will initially review all referrals.  Referrals not meeting eligibility requirements will receive notification and be referred to other appropriate agencies.
2. For applicants who meet the eligibility requirements, their information is reviewed by the Admission Committee.
3. The Admission Committee identifies the person's needs and whether or not LifeQuest can provide the services he/she is seeking.
4. Should the Admission Committee agree that LifeQuest can meet the person's needs, his/her name is added to the waiting list.
5. An opening in the appropriate environment(s) and a funding source must be secure before the person can begin receiving services from LifeQuest.

The Admission Committee will only admit those people whose needs can be met by LifeQuest directly or in cooperation with community resources or other service providers with which LifeQuest is affiliated or has contracts.
